Y Case Management

At the Y WA we are strongly committed to building healthy happy communities.

If you are a young person, aged 12 to 25 years, looking for one to one support, from accessing information, referrals, advocacy, to goal setting or informal counselling, then our non-judgemental, friendly case workers are here to help.

 

What can we help with?

Our experienced case workers provide a safe space for young people to explore and seek support for issues which affect them. Support is tailored for each young person based on their unique needs.These may include but are not limited to:

  • Goal setting
  • Education
  • Training
  • Employment
  • Accommodation
  • Drugs and Alcohol
  • Sexual health
  • Mental health
  • Sexual orientation or gender identity (LGBTI)
  • Other issues

 

Who can refer to us?

We take referrals from:-

  • Young people - you can refer yourself
  • Family/caregivers
  • Schools
  • Government agencies
  • Health professionals 
  • Community and social agencies/services

Your referral is confidential and won’t be disclosed or discussed with any other parties without the consent of the young person.

Where can you access the support?

We will see you wherever it's safe and convenient for you; it might be at one of our youth centres, or it might be at school, etc. When you contact us discuss the options with your case worker.
